Assists the patient, family, and the I.D.T in meeting the emotional, spiritual, and bereavement needs of the patient/family.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor's degree in Business or Pastoral Studies.

2. Must possess valid WV or neighboring state driver's license and must maintain WV or neighboring state minimum auto insurance.

3. Pastoral Certification in crisis management.

EXPERIENCE:

1. Experience in hospice.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Provides a safe and comfortable environment per the basic standards.

2. Ensures privacy and demonstrates concern for the dignity of the patient.

3. Completes assignments in a timely manner as per standard.

4. Works as a team member according to the standard.

5. Communicates patient care problems or potential problems to the Director and/or other IDT members as they are identified/occur to assure continuity and coordination of care.

6. Demonstrates knowledge of and adherence to patient rights.

7. Maintains patient confidentiality according to communication standard.

8. Documents patient status and care or services furnished in appropriate areas as defined by the documentation standard.

9. Documents full and accurately on progress note each visit.

10. Provides appropriate documentation of intervention, patient response, effectiveness, complications, and other patient information.

11. Provides direct and indirect bereavement and spiritual counseling to the patient and his/her family.

12. Completes assignments in a timely manner.

13. Works as a team member according to the standards.

14. Serves as Coordinator of Spiritual Care as evidenced by:

A. Coordination of Spiritual Care

B. Schedules bereavement groups and facilitates group to meet the needs of the hospice families/communities.

C. Makes initial contact 5 days after patient admission and 5 days following patient death

D. Assures bereavement follow-up as outlined in the bereavement program.

E. Serves as hospital/community contact for bereavement information.

F. Serves as a contact/resource for staff on issues in bereavement.

G. Assist Nursing homes in developing a Memorial service for patients that passed.

15. Applies positive communication skills and leadership skills when working with other clergy and outside resources.

16. States/demonstrates basic principles and practices of the major faith groups.

17. Facilitates support groups per clinical standards.

18. Provides bereaved individuals with appropriate follow-up information per policy and procedure.

19. Completes assignments in a timely manner.

20. Provides calls to newly admitted patients and family.

21. Performs public speaking as requested.

22. Identifies the stages of death and dying

22. Provides follow-up of family and/or significant others in bereavement to assist in positive expressions of grief.

6. Must be able to drive and have a valid WV driver's license.

1. Must be capable of handling information of a confidential nature.

2. Must have the ability to work successfully under highly-stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.

3. Must have the ability to comprehend and perform oral and written instructions and procedures.

4. Must have effective reading and comprehensive skills.

5. Must have strong communication skills, written and verbal.

6. Must be able to work weekends, holidays, and all shifts.

7. Must be able to prioritize tasks.

8. Must have independent decision-making ability.

9. Must have basic math skills.

10. Must have ability to respond to emergency situations in a calm, professional manner.

11.Must be capable of using computers to input and retrieve information.
