West Virginia University Research Corporation is seeking applications for a Hybrid (Part-time) ACE Program (WVU Drug Free Moms and Babies) Project Coordinator with the Department of Obstetrics and Gynecology
About the Opportunity
The Assist, Connect, and Encourage (ACE) Program is a Drug Free Moms and Babies (DFMB) Project Site at WVU Dept of OBGYN that provides additional care coordination and support for individuals with substance use disorder in pregnancy and after birth. The ACE Program is located at the Morgantown and Bridgeport/Fairmont campuses. This position is for the Morgantown location providing services to those receiving prenatal and postpartum care at clinics affiliated with WVU Children's. The Project Coordinator is a crucial member of our small team. The Project Coordinator provides direct coordination of care for ACE clients through; comprehensive assessment, referral to services and treatment, development of individualized care plans, care coordination, health education and promotion, and monitoring and follow up. They also assist in obtaining necessary consent for program participation and data collection for program evaluation. The Project Coordinator helps to promote the program through community outreach and is crucial in completing and maintaining enrollment of clients for the Maternal Opioid Use Model. This position is hybrid with telework being a large component. A work phone and laptop are provided to the Project Coordinator to help fulfill this role.
Accountability: DFMB Project Coordinator shall be employed by qualified DFMB sites as defined in the Provider Enrollment and Participation Requirements section of this policy and able to fulfill the in-person, virtual, and telephonic care coordination activities defined in DHHR Chapter 521 Appendix B. They will work under direct supervision of the health care professional or provider at the health care center.
